Chronic Conditions
  • If an athlete has a chronic condition such as asthma, we want to be sure their asthma is well controlled so they can safely participate
  • In the case of a patient with asthma, we want to ensure they have all the medications they need to safely participate, such as their Albuterol inhaler
New Injuries
  • If any new injuries or issues have arisen since an athlete’s last visit, we want to help resolve those issues prior to the sports season starting

To request an appointment, simply click the button below and you will be contacted to schedule your next steps. If you are inquiring about spine surgery, you will need to have your primary care provider make a referral to Dr. Ben Smith with Bridger Orthopedic. Spine surgery appointments are not available through the request portal at this time.
